The enhanced Personal Directives Act, which was proclaimed June 30, 2008, allows Albertans to voluntarily register their personal directive at no cost. The Personal Directives Registry is the first of its kind in Alberta because users of the registry can enter and maintain their own information.
The secure, online registry was created so that approved professionals (e.g., healthcare providers) can find out if a person has a personal directive and how to contact their agent(s) when needed. The registry does not keep a copy of the personal directive. Registration can be done online or by contacting the Office of the Public Guardian.
The registry is being introduced in two phases. As of June 30, the public can register their personal directive, and in the fall, Health Authorities and medical professionals will be encouraged to sign an agreement with the Office of the Public Guardian (OPG) to access the registry.
Registration can be done online or by mailing a Registration Form to the OPG who will enter the information. To request a Registration Form call the OPG toll free at 1-877-427-4525.
Registering a personal directive online is a two step process.
- Set up an account with Alberta Secure Access Services (ASAS). People who already have an ASAS account simply use their existing ASAS Login Code and Password. The ASAS Login Code and Password are always required to access the registry.
- Register the personal directive: an easy and self-guided process.
Once the information is submitted to the registry, a letter is mailed out by the registry to the maker and agent(s) asking if they consent to having their information on the registry. Consent is provided by using a unique enrolment code which is included in the letter. The enrolment code can be entered online or by calling the OPG.
It is possible to register a personal directive by mailing a Registration Form to the OPG. The form can be obtained by calling 1-877-427-4525 toll free. When a form is mailed to the OPG, staff will enter the information provided and the registry will mail a letter to the maker and agent(s) asking for their consent to post their information in the registry. To finalize registration, the unique enrolment code included in the letter can be phoned into the OPG or entered online.
Until the enrolment code is submitted to the registry, information on the maker or agent(s) cannot be viewed by anyone except by the staff with the OPG. If at any time in the future, a maker or agent wishes to change or remove their information from the registry, this can be done online or by calling the OPG.
